Request for Expression of Interest from interested contractors and organizations for provision of Equipment and Services in the following Areas:
The UBTH intends to undertake the procurement of various medical equipment and services for improved health care delivery in the Hospital (either wholly or through Public Private Partnership) and hereby invites application from reputable and suitably qualified companies/firms with cognate experiences to either express interest in sourcing for and providing cost effective and clinically effective or bid for Oncology Products, Magneto Resonance Imaging services, Brachy-therapy services, Laboratory Equipment and Security services under Public Private Partnership (PPP) arrangement/ supply of equipment.
Scope of Work
(1) Provision of Oncology Products
(a) Provide funds, supply and procure the needed oncology products to the Oncology Pharmacy Unit.
(b) Provide necessary technical support including training.
(2) Provision of Magnetic Resonance Imaging Services
(a) Supply and maintenance of magnetic resonance imaging equipment and accessories.
(b) Provide necessary technical support including training.
(2) Provision of Brachy-therapy Services
(a) Supply and maintenance of Brachy-therapy equipment and accessories
(b) Provide necessary technical support including training.
(4) Provision of Laboratory equipment including maintenance and training.
(5) Provision and training of Security personnel.
(6) Supply and installation of automated hormonal assay machine
(7) Supply and installation of Theatre Lights (2Nos.)
(8) Supply, installation of Electroencephalographic machine plus training
Technical Requirements
Interested contractors/companies are required to submit the following documents in the order they are listed below along with the financial bid:-
(a) Evidence of registration with Corporate Affairs Commission (CAC)
(b) Certified true copy of memorandum and Articles of Association.
(c) Last three years Company’s Audited Accounts
(d) Three years Tax Clearance Certificate.
(e) Evidence of Financial capability to execute the project.
(f) Bank reference from reputable Bank.
(g) Copies of Certified financial Statement of the Company for the last three years.
(h) Copies of VAT registration and evidence of past VAT remittance for Projects completed.
(i) Detailed information on technical personnel and key staff of the Company including their experience, registration and membership of relevant professional bodies.
(j) Specifications of the projects/equipment are obtainable from the office of the
Assistant Director (Procurement)
Financial Bid Requirements
- The financial bid document must be submitted in duplicate copies.
- Interested bidders are to make a non-refundable Fee of N30,000.00 (Thirty thousand naira) made payable to Skye Bank Plc Account No. 1770413127.
Submission of Documents
Completed technical and financial bid documents must be in two (2) separate envelopes, each appropriately labeled and sealed. The two (2) envelopes should be enclosed in a larger envelope for submission and be marked:
“Tender 2013” UBTH, Benin City at the top right hand corner indicating the Job title and addressed to:

The Expression of Interest Documents, Including the bank teller must be dropped into the box in the Chief Medical Director’s Office on or before 12:00 noon on Tuesday 26th November, 2013.
Note:
A. That the bids will be opened on 26th November, 2013 in the presence of bidders/representatives in the UBTH VIP Catering Lounge on the same day (26th November, 2013) at 12:00 noon prompt
B. That this advert serves as invitation to any Interested NGO and members of the General Public who may wish to witness the opening exercise.
C. That late submission will be rejected.
D. That the advertisement for expression of interest shall not be commitment on the part of the UBTH, Benin City nor shall it entitle any person to make any claim whatsoever and or seek any indemnity from the Hospital by virtue of such person having responded to this advertisement.
E. Only contractors who meet the technical bid requirement would have their expression of interest further evaluated.
