1. Introduction
The Presidential Committee for the Establishment of Teaching Hospital, Otukpo, for the University of Agriculture, Makurdi, intends to engage the services of interested, competent and reliable Consultants to express interest in providing consultancy services for the execution of the Special Presidential projects listed below, at the Otukpo Campus of the University of Agriculture, Makurdi.

3. Pre-Qualification Requirements
a. Interested Consultants are expected to submit comprehensive company profiles, paged with table of contents using labelled separators, to include the following, and in the under-listed order, only:
- Letter of Application.
- Evidence of Company Registration/Certificate of Incorporation from the Corporate Affairs Commission (CAC).
- Current Company Tax Clearance Certificate for the last three (3) years (2011-2013), with Tax Identification Number (TIN).
- VAT Registration Certificate with evidence of remittances.
- Evidence of personnel capability, including list of key professional staff and other support staff, photocopies of academic certificates, and signed CVs accompanied with passport photographs stapled to each CV as well as other relevant information; (passport photographs are to be authenticated at the flipside by the Principal Partner);
- Evidence of, at least, three verifiable job of similar magnitude, successfully executed (or are on-going) within the last 10 years. Experience in TETFund job shall be of added advantage.
- Evidence of registration of Principal Partners or Director(s) with relevant professional Registration Councils – ARCON, COREN, QSRBN, CORBON, TOPREC, etc.
- Evidence of current practise licence with appropriate professional Registration Councils – ARCON, COREN, QSRBN, CORBON, TOPREC, etc.
- List of relevant equipment/tools/technology capacity (appropriate software owned to be listed).
- Name, email address and mobile phone numbers(s) of contact persons.
- A letter authorising the Presidential Committee for the Establishment of Teaching Hospital, Otukpo, for the University of Agriculture, Makurdi to verify claims which are made in tender’s submission(s), with relevant agencies and bodies.
- Evidence of Employees’ Pension Funds contributions registration and remittances.
- Evidence of contributions to the Industrial Training Fund (ITF).
- Sworn Affidavit disclosing the following:
- That all information presented are true and correct in all particulars.
- That the Director(s) has/have never been convicted by any Court of Law.
- That none of the Firm’s Directors is bankrupt.
- That none of the officers of FME, NUC, FMH, TETFund or BPP is a former or present Director/Staff of the Company; and
- That the Firm is not in any form of receivership.
Applicants can indicate interest in more than one Lot, provided that each Lot is applied for separately, with all attachments also separately made.
Consortium can however make a single submission to cover all Lots of the projects. For Joint Venture, a single submission to cover all Lots is acceptable but such submission must be with all requirements under item No. 3. The Lead Partner has to be clearly indicated with a Joint Memorandum of Understanding between the Firms signed and included in the joint submissions.
4. Submission of Documents
Four (4 Nos) soft-bound copies and a soft copy of the full application (including scanned copies of all attachments) in a flash-drive/Compact Disk are required for each Lot. Submissions are to be made in sealed envelopes, clearly marked
“Expression of Interest/Pre-qualification Lot……….and Name of the Project”.

And submitted to the closed box in the office of the Secretary to the Committee, National Universities Commission, Room 213, 26 Aguiyi Ironsi Street, Maitama, Abuja.
Submission closes at 12.00 noon of Monday, 8th September,2014. Any submission made thereafter, stands rejected. The submission register must be signed with submission form filed at the point of submission.
Note:
- Pre-qualification documents should be prepared in English Language.
- Nothing in this document shall be construed to be a commitment on the part of the presidential Committee for the Establishment of Teaching Hospital, Otukpo to accept any submission or undertake the project(s) or entitle any Consultant to make any claim whatsoever, in respect of any expense incurred in preparing the responses.
- Only Firms that are pre-qualified shall be contacted.
