Capital Budget of the Hospital
Introduction:
The Federal Staff Hospital, Jabi, beside National Judicial Institute (NJI), Abuja, is a Federal Tertiary Hospital under the Federal Ministry of Health, Abuja and has received funds from the 2013 Federal Budget towards the improvement of facilities in the hospital and the Annex. Accordingly, the Hospital hereby invites interested and reputable Contractors with relevant experience and good track records to tender for the underlisted contracts, as follows: –
LOT 1: Construction of Cottage-Hospital Block at
World Royal City Cad Zone 06, Kugbo, FCT, Abuja
Lot 2: Construction of Perimeter Fence of cottage Hospital at World
Royal City Cad Zone 06, Kugbo, FCT, Abuja
2. Mandatory Requirement without Which a Bidder Shall Be Disqualified
Prospective bidders are required to submit the following documents:-
- Certificate of Registration with Corporate Affairs Commission
- Evidence of Current Tax Clearance for period of three (3) years (2010, 2011 and 2012)
- Evidence of VAT registration with TIN No. and remittances for the past three years
- Evidence of compliance with 2004 pension reform Act, and remittances
- Evidence of Training Contribution to Industrial Training Fund (ITF) in compliance with the provision of ITF Amendment Act 2011
3. Other Requirement
i. Evidence of financial capability to handle the job from a reputable Bank
ii. Company Audited Accounts prepared by a reputable Chartered Accounting Firm for the last three years (2010, 2011, and 2012) showing annual turnover Name(s) and address(es) of bankers
iii. Company profile with names and qualification(s) of key personnel with evidence of experience with similar jobs in the last three years only, with contact details of clients.
iv. Verifiable Evidence of similar jobs successfully executed in the last three years, including letters of award of contracts, project cost at completion, job completion certificates and payments (please provide contact addresses of clients who may be contacted for reference)
v. Sworn Affidavit confirming that all information presented and attached are true and correct of the company and that no officer of the service is a former or present director, staff or shareholder of the company
4.0 Collection of Tender Documents
4.1 Tender documents will be available for collection from 9:00am – 4:00pm from 17 October to 28th November, 2013 with N20,000.00 (Twenty Thousand Naira) only (non-refundable fee) during working days, paid into the Hospital’s account, as follows: –

5.0 Submission of tender Documents
5.1 The completed tender documents should be enclosed in two separate envelopes, with one marked Technical Bid Document, while the other one should be marked Financial Bid Document and placed in one big envelop and marked on the outside “Tender Documents for the LOT (Indicate the Lot Number Bidded for at the Top Right-hand Corner of the Envelop)……….. .”and addressed to the:

6.0 The completed Tender Documents should be deposited in the Tenders Box in the office of the Head of Administration of the Hospital, Federal Staff Hospital Jabi, beside National Judicial Institute (NJI), Abuja on or before 12:00noon, 28th November, 2013
7.0 Opening
7.1 Opening of the submitted bid documents will be conducted the same day (28th November, 2013) in the presence of bidders or their representatives who choose to attend at the Hospital’s conference room on the first floor, Federal Staff Hospital, Jabi, beside National Judicial Institute (NJI) Abuja, by 1:00pm the same day.
