Invitation to Tender for 2015 Main Capital Projects
Introduction:
The Management of the Federal Medical Centre, Yola wishes to invite interested and reputable Companies with relevant experience to tender for the execution of the under-listed contracts/supplies in the 2015 Financial year:
Scope of Work:
Lot 1: Furnishing & Equipping of Accident & Emergency (A&E), Amenity, Dental, Ear, Nose & Throat (ENT), Intensive Care Unit (ICU), Ophthalmology and Physiotherapy Blocks:
(a) Hospital Beds, Bedside Drawers, Over tables, Trolleys, Wheel Chairs, Couches,
(b) Office Furniture and Equipment.
Interested Contractors are hereby advised to collect the tender documents for the particular contract being bidded for from the Office of the Head of Administration upon payment of non-refundable tender fee of Twenty thousand Naira (N20,000.00) only and obtaining Official Receipt from the Finance Department after presenting Bank Draft from a reputable Bank.
Qualification:
All potential Contractors must submit together with their tenders, the following documents:
(1) Evidence of Incorporation by the Corporate Affairs Commission (CAC),
(2) Evidence of Certification/Exemption from PENCOM in accordance with the Pension Reform Act, 2004 including evidence of remittance of Employees’ Pension Contribution/Deductions,
(3) Evidence of tax clearance certificate obtained within the last recent three (3) Years,
(4) Evidence of Nigeria Social Insurance Trust Fund (NSITF) Certificate,
(5) Evidence of Industrial Training Fund (ITF) Certificate,
(6) Sworn Affidavit indicating that none of the Staff of Federal Medical Centre, Yola and Bureau of Public Procurement (BPP) is a former or present Director of the Company and has no pecuniary interest in the bidder.
(7) Evidence of Value Added Tax (VAT) Registration,
(8) The Company’s Audited Accounts for the past three (3) years (i.e. 2012, 2013 & 2014),
(9) Evidence of payment of non-refundable Tender fee of Twenty thousand Naira (N20,000.00),
(10) Full details of Company Profile including names, qualifications, Experiences of Company staff etc.,
(11) Experience/Technical qualification of key personnel (i.e, CV),
(12) Evidence of the Equipment & Technical capacity of the Company,
(13) Verifiable list of similar works successfully completed within the last three (3) years (i.e. 2012, 2013 & 2014) with names of clients, evidence of award, evidence of practical completion and completion period,
(14) Evidence of financial capability to handle the work with Company’s Statement of Accounts within the last six (6) months.
NOTE:
Only bids from Contractors who meet the above technical requirements shall be opened on the bid opening date for Consideration.
The sealed envelope should be deposited in the Office of the Medical Director, Federal Medical Centre, Yola not later than 12:00 noon 13th July, 2015. Late submission of bids shall not be accepted, please.
All Tenderers, members of the Civil Society and Non-Governmental organizations are invited to attend the financial bid opening on 13th July, 2015 at 2:00p.m. Prompt.
