Pre Qualification of Companies for the Supply of Essential Drugs/ Dressings for the Year 2015

­Background:

The Edo State Essential Drugs Programme through the Ministry of Health, Benin City, wishes to procure Essential drugs/dressings from reputable companies (major manufacturers and distributors). Interested companies are hereby invited to apply for pre-qualification to participate in half yearly bidding of Essential drugs/dressings (Please note that only pre-qualified companies will be invited to participate in tendering for 2015).

 Pre-qualification Requirements:

Interested Companies are expected to submit the following documents for pre-qualification:

(i)      Evidence of registration of the company with the Corporate Affairs Commission.

(ii)    Evidence of up-to-date registration with the Pharmacists Council of Nigeria.

(iii)    Evidence of up-to-date registration of the Superintendent Pharmacist with the Pharmacists Council of Nigeria.

(iv)    Evidence of registration with the Edo State Government (2015 Renewal of Registration with Essential Drugs Programme).

(v)     Evidence of payment of taxes to the Edo State Government.

(vi)    Evidence of settlement of PAYE.

(vii)   Evidence of similar completed jobs within the last Three (3) years.

(viii)  Evidence of Corporate Tax Clearance Certificate for the last Three (3) years.

(ix)    Company’s Profile stating: contact address including GSM number, e-mail address and website.

(x)     Evidence of financial capability.

Submission:

Pre-qualification documents are to be addressed in sealed envelope and submitted to the Programme Manager, Edo State Essential Drugs Programme, Medical Stores Complex, No. 4 Medical Stores Road, Benin City not later than 16^th February 2015.
