Invitation for Pre-Qualification and Tendering
1.0 Introduction
The Cross River State Department of Community Health/Primary Health Care, in collaboration with Society for Family Health and support from the Nigerian Governors’ Forum to implement projects supported by the Leadership Challenge Grant Awards from Bill and Melinda Gates Foundation (BMGF) hereby invites interested, competent and reputable contractors to apply for tenders for the proposed under listed projects:
2.0 Construction of a 4 room bed seater block (self contained apartment) in the following health facilities across the State
LOT 1: Southern Senatorial District
Akamkpa LGA-PHC Aningeje
Akpabuyo LGA-PHC Ikot Offiong
Ambai Bakassi LGA-PHC Ikang
Biase LGA- PHC Ikot Ana
LOT 2: Central Senatorial District
Boki LGA-CHCOkundi
Ikom LGA- PHC Agbragba
Obubra LGA- PHC Ochon
Yakurr LGA- PHC Mkpani
LOT 3: Northern Senatorial District
Bekwarra LGA- Model PHC Abochiche
Obanliku LGA- PHC Shikpeche

Yala LGA- PHC Ebo Ipuole
3.0 Procurement and production of Traditional Birth Attendants (TBA) Delivery Kits.
LOT 4: Procurement of delivery instruments to produce 1,200 TBA delivery kits.
4.0 Eligibility
Reputable contractors are required to submit their application along with the following documents:
- Evidence of Legal status of Organization which includes the following
a. Corporate Affairs Commission form C02
b. Corporate Affairs Commission form C07
c. Certificate of Incorporation
2. FIRS Tax Identification Number (TIN) and evidence of current CRS tax clearance certificate
3. VAT Certificate of Organization and evidence of VAT remittance in the past three (3) years
4. Organization’s Banker’s reference letter
5. Evidence of Prior experience of organization in desired area of work including letters of award, practical
completion certificate etc within the past three (3) years
6. Current list of clients and references
7. Evidence of registration with CRS Due Process Office
8. Evidence of financial capability and banking support
9. A Comprehensive Company Profile which should include names, qualifications and experience of key personnel
10. Company’s Audited Account for the last three (3) years prepared by a Chartered Accounting Firm
11. Sworn affidavit in line with the provision of part IV section 16, subsection 6 of the Procurement Act 2007.
5.0 Collection of Bid Documents
The bid documents are obtainable from the Society for Family Health – State Embedded Technical Consultant (SETC) at the Department of Community Health/Primary Health Care, No. 30 Barracks Road, Calabar on the payment of bid document fee of Five Thousand Naira (N5,000.00) only.
6.0 Tender Fee
Tender document should be submitted with the original receipt of payment of non refundable tender fee of Fifty thousand Naira (N50, 000.00) only per project, made payable to Govt. Account- Department of Due Process and Price Intelligence.
7.0 Submission of Bid Documents
All completed pre-qualification and tender documents must be filled in ink and submitted in sealed envelopes and labeled (Technical and Financial bids as appropriate) separately, and the two separate envelopes put in another envelope that should also be sealed and boldly marked,” Tender Requirements for LOT. 8.0 Closing of Submission
Submission of tenders commences on the 4th August, 2014 and closes on 18th August 2014 by 12noon prompt.
9.0 Opening of Bid
All pre-qualification bids will be opened at 2.00pm on 18th August 2014.

- Late bids will be rejected.
- Only financial bids of pre-qualified bidders shall be opened for the tendering exercise.
