Invitation to Pre-Qualification of Vendors
Center for Integrated Health Programs (CIHP) is a Non-Governmental Organization implementing HIV/AIDS care and treatment programme in Nigeria. We regularly procure the under listed materials in order to service the day-to-day requirements of our supported health facilities. To this end, interested and competent companies are hereby invited to submit their company/business profiles for Pre-qualification.
LOT 1: Clinical Services Items; including but not limited to:- Examination couch, wheel chair, examination screen, diagnostic set, Delivery kits, stethoscope, Gallipot, Kidney Dish, Motorised patient stretcher, tongue depressor, sphygmomanometer, examination torch, thermometer, delivery couch, Pharmaceutical supplies (Dispensing Pill envelopes, Pill cutters, Preventive jobs aids (Penile model), home based care kits (Water buckets, Treated nets, and water purifiers) etc (Bids should include brand/manufacturer names).
LOT 2: Printing of Patient Monitoring & Management (PMM) Forms and Behavior Change & Communication (BCC/IEC) Materials; e.g.
Registers: A4, A3 and customised sizes, Carbonized forms, documents of bond paper and loose sheets, colored fliers, leaflets & handbills of different sizes, PMM Registers, books, (with graphics), branded T – shirts, face caps and other materials etc.
LOT 3: Office furniture. Electronics, and equipments;
Included but not limited to; Office tables, chairs. Generators, Electronics (TVs; Tape Recorders for HCT & BCC activities), waiting benches of different sizes and designs, Laboratory work tops/cabinets, Metal and wooden shelves, office drawers and cabinets etc.
LOT4: Medical/Laboratory Consumables
All ranges of Medical/Laboratory Consumables; Methylated Spirit, Cotton wool, Sodium Hypochloride (Local Bleach) Absorbent Cotton rolls, Examination gloves, X-ray films, reagents, Leishman Stain, Methanol, Serology Tubes, etc. (Bids should include brand/manufacturer names).
LOT 5: IT Equipments/ IT solutions: Office Consumables
Computers (Desktops and laptops & accessories) , printers, scanners, photocopying machines; web designs and developments; IT infrastructures; Internet Service Provisions; Toners for Copiers, Stationeries, tissues, pins, clips etc. (Bids should include brand/manufacturer names).
LOT 6: Office Utilities
Office utilities; Diesel and Gasoline, Water, routine services and repairs of equipment, etc (Bids should include brand/manufacturer names).
LOT 7: General Supplies:
General but non classified items including but not limited to: Food bank items, Child/youth friendly items (Games, cards, play pens, child educational materials)
LOT 8: Laboratory Equipments
Laboratory machines and equipments for various tests – CD4, Hematology, Chemistry. Microscopes for microscopy; cold chain equipment like cold/vaccine boxes, refrigerators; blood banks etc
LOT 9: Facilities Repairs
Minor repairs in supported facilities/offices spread across Abuja, Kaduna, Kogi, Gombe, and Benue states
LOT 10: Vehicles & Automobiles
All categories of automobile equipment (Cars; Trucks; Motor Bikes etc) for mobility of project staff and project activities in supported institutions
LOT 11: Third Party Logistics Services (3PLs)
Clearing and delivery of shipments from ports; warehousing services for project commodities in Abuja, Kaduna, Gombe, Benue & Kogi States; Transportation of project commodities from Abuja to various locations in the 4 states listed above (Kaduna, Kogi, Benue & Gombe)
LOT 12: Drugs
Drugs for opportunistic Infections in HIV/AIDs: Antimalarial; Multivitamins.
Pre-Qualification Criteria:
The Prequalification criteria will include below listed- However, some of the criteria are lot specific criteria and its required that interested companies addresses both general requirements and lot specific requirements.
- Evidence of company registration with the Corporate Affairs Commission Including certificate of incorporation; farms CAC 2, and CAC 7, Article and Memo of incorporation.
- Company profile, including name, verifiable address, active telephone numbers, active email addresses and organization’s administrative structure.
- Tax Identification Number (TIN) with evidence of tax remittance (Tax clearance certificate) for immediate past three years
- Evidence of VAT registration with evidence of VAT remittance
- Evidence of bank account in the name of the company/business entity
- Verifiable list of similar jobs successfully executed in the past (inclusion of letters of award and completion certificates an added ad-vantage).
- For companies that 3re manufacturers representatives, evidence of authorization as manufacturers sole representative or major distributor.
- For manufacturers of drugs or manufacturers representatives, evidence of retention of manufacturing or practicing premises, annual practicing license of the superintendent pharmacist; evidence of NAFDAC registration for the product(s) being
- Active email address in the name of the company
Prequalification Guide:
Interested companies/business entities applying for pre-qualification should note the following:
(1) That CIHP reserves the right to reject any or all pre-qualification
Submissions.
(2) That CIHP will not enter into correspondence with any company
on why the company was pre-qualified or not pre-qualified.
(3) Response to this invitation exercise should not be construed as a commitment on the part of CIHP, nor shall it entitle any company to make any claims whatsoever and /seek any indemnity from the organization.
Completed pre-qualification documents to be submitted in sealed envelopes and marked at the bottom right corner indicating the Lot & title of interest, and addressed to:
