Invitation to Tender for the Execution of Projects in the Hospital
(1.0) Introduction
University of Benin Teaching Hospital, a leading health care service provider in Nigeria hereby invites qualified and reputable contractors/suppliers to bid for the execution of some projects in the Hospital,

(3.0) Eligibility Criteria
Firms which are interested in bidding for jobs must possess the following eligibility requirements:
(i) Certificate of registration with the Corporate Affairs Commission (CAC)
(ii) Detailed Company Profile
(iii) Tax Clearance Certificate for the last Three (3) years (2012, 2013 and 2014).
(iv) Evidence of VAT compliance for the last Three (3) years including VAT Certificate.
(v) Evidence of registration with PENCOM/ remittances.
(vi) Evidence of registration with ITF as well as remittances.
(vii) Audited company account for the last Three (3) years (2012, 2013 and 2014).
(viii) Evidence of similar jobs/assignments completed/on-going by the company in the past.
(ix) Evidence of financial strength and credit-worthiness and solvency from company’s Banker(s).
(x) A sworn affidavit in line with the provisions of Part IV, Section 16, Subsection 6(f) of the Public Procurement Act, 2007 stating that:
(i) All documents/information presented are genuine and verifiable.
(ii) The Director(s) of the firm has/have never been convicted by any court of law within or outside Nigeria.
(iii) None of the company’s Directors or the company itself is bankrupt,
(iv) None of the officers of University of Benin Teaching Hospital (UBTH) or Bureau of Public Procurement (BPP) is a former or present staff of the company.
(v) The firm is not in receivership.
These documents are to be arranged in the order in which they are listed.
(4.0) Collection of Bidding Documents
Interested companies are to collect the Tender Documents from the Procurement Division (Room 8) Administrative Block, (Laundry Extension) from the date of publication of this advert between the Hours of 9 00am and 4 00pm daily with the evidence of payment of a non-refundable fee of N30,000.00 (Thirty Thousand naira) only per lot. The sum is to be paid Into UBTH Account number 1770413127 with Skye Bank Plc, UBTH premises, Benin City and a photocopy of the receipt presented before the collection of Tender documents.
(5.0) Submission of Bidding Documents
Tender documents are to be enclosed in two separate envelopes, one to contain the Technical Bid which must be boldly marked “Technical Bid” on the top right corner of the envelope, while the second envelope is to contain the Financial bid which is also to be boldly marked “Financial Bid” on the top right corner of the envelope. The two envelopes are to be placed in one large envelope which should be addressed to:

University of Benin Teaching Hospital,
P.M.B 1111,
Benin City,
Nigeria.
The lot number of the job bidded for should be written on the top right corner of the front of the envelope. Also the name of the firm should be written boldly on the other side of the envelope.
The envelopes are to be submitted in a Tender box at the Procurement Division (Room 12) Administrative Block, (Laundry Extension) after they have been registered. All Tender documents are to be submitted not later than 12,00noon on Tuesday 12th May, 2015. Bids submitted after the deadline for submission shall be rejected.
All submissions are to be made in English Language.
(6.0) Opening of Bidding Documents
The bids shall be opened immediately following the deadline for submission on Tuesday 12th May, 2015, by 12.00 noon at the VIP Lounge, Catering Department, UBTH, Ugbowo, Benin City. Bidders or their representatives, as well as interested members of the public are invited to witness the opening of the bids.
(7.0) Disclaimer
The University of Benin Teaching Hospital is not obliged to award the contract to the lowest cost bidder.
