Invitation for Pre-Qualification and Tender
1.0 Introduction
The National Orthopeadic Hospital, Dala – Kano invites Tenders and Pre- qualification Documents from competent and experience Contractors/Suppliers for execution of the Hospital’s Capital 2013 Project.
Lot 1. Supply of Medical Equipments for the Operating Theatre.
2.0 Evaluation Requirements
1.0 Evidence of registration with Corporate Affairs Commission (C.A.C)
2.0 Evidence of Tax Clearance for the last three (3) years.
3.0 Evidence of VAT Registration and Remittance.
4.0 Evidence of Compliance with Pension Reforms Act 2004 with regards to operation of the employees Retirement Savings Accounts with an approved Pension Fund Administrator (PFA).
5.0 Company Profile with Evidence of Personnel capability including list of key personnel, C.V of each professional.
6.0 Evidence of experience in jobs of similar nature within the past three (3) years with verifiable letters of contracts award or completion certificate.
7.0 The Company must provide evidence that the Medical Equipment meet International Standard with appropriate guarantee.
8.0 Evidence of financial capability to handle the job by way of turnover figures backed by three years Audited Report. Bank statement and Bank reference letter.
9.0 Evidence of compliance with Provision of Section 6 (i-iii) of the Industrial Training Fund (ITF) amended Act no. 19 of 2011 with regards to payment of contribution for employees training.
10.0 Evidence of registration with professional bodies such as ARCON, COREN, SURCON, etc.
Interested companies are to obtain Tender/Bid Documents from Planning Unit of Administration Department of the Hospital from 4th November, 2013 to 13th December, 2013 upon the presentation of the receipt of payment of non-refundable Tender processing fee of N5,000:00 of the Lot.
Only the submissions which fulfill the requirement stated above will be considered.
3.0 Submission
Technical and Financial documents should be package in separate enveloped identifying the contents as either Technical or Financial documents, sealed and addressed to:

The two envelopes are to be put in a bigger envelope addressed also as above and clearly list she content, submission are to be deposited in the Tender Box provided in the Office of the Medical Director on or before Friday 13th December, 2013 by 12.00 noon when the proposals and Bids will be open immediately in the Hospital Board Room.
Tenders/Bidders are to note that the Hospital is not bound to accept the lowest or any bid.
Tenders/Bidders are also to note that wrong package or labeling will result in disqualification.
The Tender/Bid opening event is open to interested members of the public.
