Invitation to Tender for Year 2014 Capital Projects
Preamble
Applications are hereby invited from qualified Contractors to tender technical and financial bids for the following projects which will be funded through 2014 Capital Appropriation and Internally Generated Revenue (IGR).

Eligibility Criteria
Interested Contractors/Companies are required to submit the following documents in sequential order as listed below:
(a) Certificate of Registration with Corporate Affairs Commission (C.A.C).
(b) Company’s Tax Clearance Certificate for the last three (3) successive years (2011, 2012, and 2013).
(c) Evidence of VAT Registration.
(d) Evidence of Certificate/Exemption from PENCOM in accordance with Pension Reform Act (2004) including evidence of remittances of Employees Pension Contributions/Deductions.
(e) Evidence of Registration/Payment of Training Contribution in compliance with the Statutory Provision of the Industrial Training Fund ACT (2011) Amended.
(f) A sworn Affidavit that no officer of Neuropsychiatric Hospital, Aro, Abeokuta is a former or present Director, or has any pecuniary interest in the bid.
(g) Evidence of financial capability to execute the project (e.g. Current Bank Statement certified by Company’s Banker) and/or Banking Support.
(h) Verifiable list of similar jobs successfully executed in the last three (3) years including letters of award of contracts, project costs, Job completion certificates etc.
(i) Company’s Audited Account for 2011, 2012 and 2013.
(j) Company’s Profile Including Qualifications of key personnel and equipment.
(k) Any other additional information that may enhance the company’s application.
Note:
Documents (a) – (f) are Responsive Criteria, the absence of any automatically disqualifies the bidder.
Submission of Bids
Technical and Financial Bids are to be Separately Sealed with wax in envelopes indicating Lot number/title, name and address of the Bidder at the top left corner of the envelope with the Bidder’s GSM Number/Reachable Number.
All documents should be submitted on or before Tuesday, 30th September, 2014 by 9:00am when bids will be opened.

Please note:
(1) The bids must be dropped in the Bid Box located in the Office of the Provost and Medical Director
(2) All submissions should be by hand. Courier delivery win not be considered.
(3) Late submission will not be considered.
(4) Only companies that pass technical evaluation will have their financial bids opened. The Hospital reserves the right to reject any or all tender documents.
(5) Please visit the Procurement Unit of the Hospital for further information.
All Tenderers, Civil Society Organizations and interested Stakeholders are invited to the tender opening scheduled to take place immediately after the close of submission on Tuesday, 30th September, 2014 in the Boardroom, Postgraduate Building, Neuropsychiatric Hospital, Aro, Abeokuta.
