Invitation to Tender for Supply of Medical Equipment
1. Introduction
The Olabisi Onabanjo University Teaching Hospital Management Board is desirous of undertaking some projects and would require the services of consultants/contractors to ensure quality assurance in the job. The projects are as follows:
Lot I: – Supply of Medical Equipment
Lot II: – Renovation and Rehabilitation of some Physical Structures in the Hospital
(A) Renovation and Expansion of the Children Emergency Ward
(B) Rehabilitation/renovation of old Cafeteria Block to incorporate offices
(C) Modification of the present New Cafeteria Building to a two storey building incorporating offices and clinics.
2. Scope of Works
Lot I
Supply of Medical Equipment and installation where applicable
Lot II
a. Architectural works
b. Structural works
c. Mechanical/Electrical works
d Quantity Surveying
3. Scope of Consultancy Services Lot II:
3.1 Preparing drawings [Architectural, Structural, Mechanical/Electrical].
3.2 Preparation of Bill of Quantity.
3.3 General Supervision of the works.
3.4 Preparation of Site Progress Report.
3.5 Preparation of Interim payment (where applicable) which constitute Authority for all payments to the Contractors by the Hospital;
3.6 The Hospital Contractor and Consultants must hold periodical meeting to the report and also draw up project plan.
4. Eligibility Criteria
Interested suppliers/contractors are required to submit their company’s documents in sequential order as listed:
(a) Certificate of Registration with Corporate Affairs Commission.
(b) Company’s Tax Clearance Certificate for the last three successive years (2011, 2012, 2013).
(c) Evidence of VAT Registration or Tax Identification Number.
(d) Evidence of Registration with Relevant professional bodies, if applicable;
(e) Evidence of financial capability to execute contract and bank guarantees
(f) Business profile if applicable.
(g) Verifiable list of similar job successfully executed in the last three (3) years including letters of award, activity description, client, location, value, status (complete or on-going, etc.)
(h) Company’s profile including the qualification of key personnel and business.
5. Tender Documents can be Obtained from: Office of the Director of Administration, Olabisi Onabanjo University Teaching Hospital (oouth) Sagamu,
upon payment of non –refundable fee of N10,000 for Lot I and N25,000 for projects listed on Lot 2. Tender fee[s] payment to be made to Olabisi Onabanjo University Teaching Hospital at Guaranty Trust Bank Account Number: 0049890638.
Note
Lot I:
Must be Submitted on or before Tuesday 22nd April, 2014 At 12.00noon when the tender will be opened.
(i) Documents [a] to [g] are mandatory criteria; the absence of any automatically disqualifies the bidder. Bidders may bid for one or more Lots as award would be conducted on Lot and Lot basis.
(ii) Bidding for incomplete Lot will be considered non-responsive and will therefore be rejected.
(iii) Fully completed tender documents should be enclosed in sealed envelopes indicating the Lot number at the top right corner of the envelope and addressed to:

Not later than Wednesday, 23th May, 2014 at 12.00noon.
6. Bids will be opened in the presence of bidders or their representatives at 12.00 on Wednesday, 23rd May, 2014.
7. General Information
This advertisement shall not qualify any tender/bidder to make any claim whatsoever, and or seek any indemnity from OOUTH or any of its officers by virtue of their participation in this tender.
