Invitation to Prequalify and Tender for the Execution of 2014 Capital Projects
The University College Hospital, Ibadan, Nigeria’s foremost teaching Hospital hereby invites interested and reputable contractors, suppliers and manufacturers with relevant experience and good track record to pro-qualify and tender for the under listed.

LOT 9 | Rehabilitation/Reconstruction of the Sewage Treatment Plant within the Hospital |
Method Of Application
Interested Contractors, Suppliers or Manufacturers should submit five (5 Nos.) copies of their detailed proposals with the following, as these will be used for pre-qualification. A comprehensive and paged Table of Contents, providing information on the company profile among other requirements should be presented in the following order:
a.) Letter of application
b.) Certificate of registration with the Corporate Affairs Commission (CAC)
c.) Copies of immediate past three (3) years Tax Clearance Certificates with Tax Identification Number (TIN)
d.) Evidence of three years (3) years Company’s Audited Report
e.) List of key personnel including their professional registration certificate, their CVs with passport photograph endorsed by the company’s Chief Executive
f.) List of key equipment;
g.) Evidence of financial capability and bank support;
h.) Evidence of VAT registration and remittance;
i.) Verifiable evidence of at least five (5 Nos.) similar projects handled in the last seven (7) years (indicating the clients, contract value and location of the projects with evidence of completion certificates or IPCs, if on-going);
j.) Evidence of compliance with the ITF Amendment Act, 2011 by including the Letter of Compliance from the Industrial Training Fund;
k.) Evidence of compliance with Pension Reform Act, 2004 by including the Letter of Compliance from the National Pension Commission;
l.) A letter authorizing the University College Hospital, Ibadan to verify all claims with relevant agencies and bodies;
m.) Sworn Affidavit in line with the provisions of Part IV, Section 16, subsection 6 (I) of the Public Procurement Act, 2007, which should include among others that:
- all information presented are true and correct in all particulars’
- that the Directors) of the firm has/have never been convicted by any Court of Law;
- that none of the company’s director or the company itself is bankrupt;
- that none of the officers of UCH, Ibadan or BPP is a former or present director/staff of the company;
- that the firm is not in receivership
Method of Paying For Tender Documents
Interested suppliers/contractors should pay a non-refundable tender fee of N30,000.00 (Thirty Thousand Naira) only into the Hospital’s Tenders Account at the First City Monument Bank (FCMB) Plc, UCH Branch and convert the bank Teller to a receipt at the Hospital’s Finance & Accounts Department. Photocopy of the receipt should be attached to the Tender Documents.
Submission of Pre-Qualification and Tender Documents
Submission of documents is to be done in two separate envelopes clearly marked at the top left corner as “Prequalification/Technical Bid (of the applicable LOT) FOR YEAR 2014 Projects” and as “Financial Bid ( of the applicable LOT) for year 2014” with the company’s name at the bottom left corner for easy identification. The completed prequalification and tender documents should be enclosed in an envelope, sealed and marked appropriately in CAPITAL LETTERS AT THE LEFT CORNER and addressed to:

The completed and sealed tender documents should be deposited in the Tenders Box in the office of the Deputy Director (Procurement) not later 12:00 noon on Monday 30th June 2014. Any submission made thereafter stands rejected.
Opening
The submitted documents will be opened on the 30th June 2014 at 12.00noon at the Omolade Alade Hall of the School of Nursing, UCH, Ibadan. Bidders are required to be present at die opening.

3. Companies that have current Technical Agreements with manufacturers are required to submit copies of such Agreements in lieu of providing a list of key personnel.
