Tender for the Execution of the Underlisted Projects (Lots 01 – 03) in the Hospital
Tenders are hereby invited from suitably qualified and registered Contractors with the Hospital for the following projects
SECTION A (CONSTRUCTION)
Lot 01: The Renovation of the Accident & Emergency Unit of the Hospital.
Lot 02: The Renovation of the Private Rooms of the Hospital.
Lot 03: The Construction of Diagnostic IVF Suites.
Lot 04: Lead-Lining of CT Scan Suite in Radiology Department
SECTION B (HOSPITAL EQUIPMENTS)
Lot 05: CT – Scanner (specification – TOSHIBA).
Lot 06: Digital X-Ray Processor (specification – KODAK).
Lot 07: Flexible Bronchoscope =Fibreroptic= (specification – PENTAX, FUJNON OR OLYMPIC).
Lot 08: Laparoscope + Energy Source and Accessories for minimal access surgery (specification – KARL STORZ).
Lot 09: Operative Hysteroscope and Accessories (specification – KARL STORZ OR OLYMPUS)
Lot 10: IVF facility Unit (Equipment, accessories and consumables).
Lot 11: Electrocovulsive Therapy Machine (specification – Electronustim & Constant Current Series 6 ECT Machine, London Product).
Lot 12: Keratomerer + Biometry Machine + Biometry Machine =IOL Master= (specification CARL ZEIS).
Lot 13: Wat Field Bipolar Coutiery x 2 (specification – ELLIS OPHTHALMIC TECH. INC).
Lot 14: Electric Drill (specification – AESCOLAUP).
Lot 15: Stress ECG.
Interested contractors are expected to submit along with their financial bids the following documents:
(1) Certificate of registration of your firm with the Corporate Affairs Commission.
(2) Evidence of last three (3) year’s Tax Payment Certificate.
(3) Company profile to include names, qualifications and experience of key staff and their technical competence.
(4) Evidence of similar job(s) done elsewhere indicating present status of the contract.
(5) Evidence of verifiable financial capability from the firm’s bankers.

The bid which closes at 10:00am on 1st April, 2015, should be submitted in sealed envelope and marked with the appropriate caption e.g. “Tender for Renovation of the Accident & Emergency Unit (LOT 01)” to the Office of:
