Expression of Interest (EOI) For Engagement of Consultants
The University College Hospital (UCH), Ibadan, Nigeria’s foremost teaching Hospital, intends to engage the services of interested, competent and reliable consultants to express interest in providing consultancy services for the execution of the Year 2014 Capital Projects listed below:
Summary of Scope
Lot 1 – Construction of Cardiac Building
Lot 2 – Construction of Water Treatment Plant
Lot 3 – Renovation of 18 Wards (Phase 1)
Lot 4 – Extension, Renovation and Equipping of the Blood Bank Centre (Phase I)
Lot 5 – Construction Of sickle Cell centre
Lot 6 – Rehabilitation/Reconstruction of the Sewage Treatment Plant within The Hospital.
EOI Requirements
Interested Consultants are required to submit the following documents:
a.) Certificate of Registration with the Corporate Affairs Commission (CAC)
b.) Copies of immediate past three (3) years Tax Clearance Certificate with Tax identification Number (TIN)
c.) Evidence of VAT registration and remittance
d.) Evidence of three years of (3) years Company’s Audited Report
e.) Detail of staff strength including the curriculum vitae and profile of key personnel
f.) Evidence of membership of professional Training Association
g.) Evidence of financial capability and bank support
h.) Verifiable evidence of previous consultancy experience undertaken in the last five (5) years including scope of work and value
i.) Evidence of compliance with the ITF Amendment Act 2011 by inclusion of Letter of Compliance from the Industrial Training Fund
j.) Evidence of compliance with the Pension Reform Act, 2004 by inclusion of Letter of Compliance from the National Pension Commission
k.) A letter authorizing the University College Hospital (UCH), Ibadan to verify claims with relevant agencies and bodies.
Successful consultants will be selected in accordance with the procedures set out in the Public Procurement Act 2007 and the Procurement Procedure Manual published by the Bureau of Public Procurement (BPP).
Submission
The submission is to consist of one original and two (2) duplicate copies in a sealed envelope marked “EOI FOR THE ENGAGEMENT OF A CONSULTANT” (specify Lot) and addressed to;

University College Hospital, Ibadan
The sealed envelope containing the “Expression of Interest” should be deposited in the Tenders Box in the office of the Deputy Director (Procurement) not later than 12:00 noon on Tuesday, 15th July 2014 and any submission made thereafter stands rejected.
Opening
All submitted EOI would be opened on the same day, Tuesday, 15th July 2014 immediately after the closing time (12:00 noon) at the Omolade Alade Hall of the School of Nursing, UCH, Ibadan. Consultants or their representatives are expected to be present at the opening.

- Pre-qualified consultants will be requested to pay =N=20,000.00 processing fee before collecting the Request for Proposal (RFP).
- The Hospital is not bound to shortlist any consultant, and reserves the right to annul the selection process at any time without incurring any liabilities and assigning any reason thereof.
