Expression of Interest for the Management of Anambra State University Teaching Hospital, New Mortuary
The Anambra State University Teaching Hospital Management Board hereby invites tenders from competent and interested individual and companies who will manage its modern mortuary.
Requirements:
Tenderers should provide evidences of the following:
a) Name of Company
b) Certificate of Registration of the company as morticians
c) List of staff, stating their qualifications as morticians and number of years of experience
d) Evidence of working experience in mortuaries
e) Evidence of current license to practice as morticians
f) Indication of contract fee and terms of payment
g) State type of preservation you undertake
h) Evidence of payment of Tax for the last 3 years
i ) Attach two referee reports to attest of your ability to manage a modern mortuary.
Payment of Non-Refundable Fee
Interested companies shall pay a non-refundable tender fee of N10,000 in certified bank draft payable to Anambra State University Teaching Hospital, Amaku-Awka.
Submission of Tender
The tender must be seated in an envelope and marked on the top ten hand corner with inscription “Tender for Management of Hospital Mortuary” and addressed to the “Ag. Chief Medical Director, Anambra State University Teaching Hospital” to reach him on or before 19th March 2014. Only shortlisted applicants will be invited for interview.
