Invitation for Prequalification
Introduction
Government of Ekiti State, Nigeria, in the pursuit of delivering secondary level specialized health care services through her newly constructed 250 bedded Oba Adejugbe Hospital, intends to pre-qualify competent firms with proven experience in Medical Technology and Healthcare Management for the supply, installation and management of medical equipment in her ultra-modern hospital.
2. Eligibility Criteria
The pre-qualification documents from interested organizations should include the following:
- Evidence of registration with CAC
- Company Tax Clearance Certificate for the last 3 years
- Company Audited Accounts for the last three years
- VAT registration
- PENCOM payments evidence
- Evidence of Payments to ITF
- Evidence of Financial Capabilities from reputable Bankers
- List of key personnel including Managerial, Technical and Administrative personnel
- Evidence of Distributorship with OEM (S)
- Evidence of Performance of similar assignments
- VAT registration
- Registration with Ekiti State Bureau of Public Procurement
3. Pre-Requisite for Processing of Pre-Qualification
Payment of non-refundable fee of NGN N100, 000.00 (One hundred thousand naira) only in bank draft made payable to Ekiti State Ministry of Health, and collection of prequalification documents at the address below:

4. Submission of Documents
Pre-qualification documents bounded in sealed envelope with title and senders address clearly written at the top right hand corner and addressed to:
The Secretary,
State Tenders Board,
C/o Cabinet and Special Services Department,
Office of the Executive Governor,
Ado-Ekiti.
to be submitted at the State Tenders Board (STB) at Governor’s Office Ado-Ekiti, Ekiti State. Contact Telephone: +2347065864713 and hand delivered into the pre-qualification box in the office of the Secretary State Tenders Board not later than March 28th 2014.
