Request for Proposal (RFP)

A.      Introduction

Family Health International (FHI 360) with funding from the United States Agency for International De­velopment (USAID) is supporting the government of Nigeria (GON) to enhance sustained cross-sec­tional integration of HIV/AIDS services by building Nigerian capacity to deliver sustainable high quality, comprehensive prevention, treatment, care and related services in various states within Nigeria.

The Nigeria Country Office hereby qualified organizations to submit proposals for the services speci­fied in Section B (List of Required Services) below

B.      List of Requirements

The scope of work this supply involves delivery of various laboratory consumables as listed below.

C.      Eligibility Criteria

The technical competence of bidders will be evaluated before the financial offers are considered. The minimum criteria that must be met by successful bidders are as follows:

1.       Evidence of registration with Corporate Affairs Commission and other bodies with copies of form CO7 & CO2

2.      Current/valid Tax Clearance Certificate (expiring December 31, 2014)

3.      Detailed Company Profile with registered address, functional email, telephone numbers and point of contact for the organization.

4.       Verifiable evidence of similar jobs successfully executed in the past for an amount equal to or above 14 million Naira (including copy of job order and signed delivery note)

5.      Recommendation Letter from the Bank stating ability to execute such supply.

6.      Current company’s audited statement of accounts for the last three years (2011, 2012, and 2013)

7.       Affiliation with any international organization or original equipment manufacturer

8.      Provide recommendation letter of supply from past Organization for successful completion of supply orders

9.       Vendors with in-country representation for after sales services will have an added advantage

Bidders are advised to submit all documents in the order listed above. Failure to do so may lead to disqualification.

D.      Submission of Proposals

Bidders are requested to submit technical and financial bids in separate envelopes All financial bids must be in Naira (NGN) and signed by an authorized representative of the bidding company. Financial bids must include the following:

1. Delivery Schedule
2. Payment terms
3. Validity of Quote
4. Warranty of Equipment

All documents required by this RFP must be received in a sealed envelope and clearly labeled Labo­ratory Supplies/2014 at the above designated location No Later Than 5:00pm, Tuesday 19^th  May, 2014

No Facsimile (fax), e-mail documents or postmarked documents will be accepted. Issuance of this RFP in no way constitutes a commitment by FHI360 to award a contract.

Bidders are to further note that:

• All Submitted Proposals must be Valid for a period of 90 days from the closing date indicated Above.
• FHI 360 reserves the right to vary number of items at time of award or split the award as may be deemed necessary in terms of Quantity to be delivered.
• FHI 360 may request successful Bidders to provide physical sample of items they are been considered for supply.
• FHI360 is Value added tax (VAT) exempted, but charges Withholding Tax (WTH) in compliance with the relevant Nigerian laws.
