Addendum

The Abubakar Tafawa Balewa University Teaching Hospital, Bauchi wishes to notify our prospective bidders of our earlier advertisement on pre- qualification which appeared in the Guardian and Leadership News papers of Friday 24th October, 2014 that items 1,2,4,5 & 10 are corrected thus:

Requirements

(1)     Evidence of Compliance Certificate of Incorporation and Registration with the Corporate Affairs Commission (CAC).

(2)     Evidence of Company Tax Clearance Certificate for the last three (3) years (2011, 2012 and 2013).

(3)     Evidence of Current Pension Clearance Certificate from PENCOM.

(4)     Evidence of Compliance Certificate with the provisions of Section 6(1) (3) of the amended Industrial Training Fund (ITF) Act No. 19 of 2011.

(5)     Audited Account for the past three (3) years (2011,2012 and 2013) and

(6)     Evidence of Registration with the Nigeria Security Civil Defence Corps (NSCDC)

Closing date for the submission of pre-qualification document is extended to 8^th December, 2014 by 12:00 pm and opening will be at same time on the same day.
