Invitation for Pre-Qualification of Health Maintenance Organizations (HMOS) and Tenders for the Execution of Tertiary Institution Social Health Insurance Programme (TISHIP) at Usmanu Danfodiyo University, Sokoto
(1.0) Introduction
The Usmanu Danfodiyo University, Sokoto intends to take a Health Insurance cover for all students of the University under the Tertiary Institutions Social Health Insurance Program (TISHIP) of the National Health Insurance Scheme (NHIS) and hereby invites reputable Health Maintenance Organizations (HMOs) to submit pre-qualification documents for assessment.
(2.0) Scope of the Programme
To provide Administrative, Financial, Technical and other Professional services that will ensure high standards of health care delivery services for students of the University both during session and on holidays.
(3.0) Pre-Qualification Requirements
Prospective HMOs wishing to participate are required to submit to their pre-qualification bids containing the under-listed documents:-
(a) Certified true copy of certificate of registration with (NHIS).
(b) Certified true copy of certificate of registration with Corporate Affairs Commission (including form CO7).
(c) Certified true copy of company’s Tax Clearance Certificates for the last three years (2012-2014).
(d) Audited Accounts of the company for the last three (3) years (2012 – 2011).
(e) VAT registration and evidence of remittances/exemptions in the last 3years.
(f) Evidence of remittance of training contribution in compliance with FTP ACT, 2011.
(g) Evidence of remittance of staff pension contribution to toe appropriate Pension Fund Administrator.
(h) List of all administrative, financial, technical and information technology staff, their qualifications and experience, including Board members and Financial Controller.
(i) List of similar and verifiable institutions being bandied for TISHIP by the HMO.
(4.0) Submission of Pre-Qualification Documents
The completed pre-qualification documents must be submitted in sealed envelope marked “TISHIP BID”, addressed to:

(5.0) Payment of Non-Refundable Fees
Interested HMOs are to pay a non-refundable pre-qualification processing fee of fifty thousand (N50,000.00) naira only and show evidence of payment when submitting their documents. All payments should be made into Usmanu Danfodiyo University, Sokoto. Account No. 1751075715 at any branch of Skye Bank, Plc.
(6.0) Closing Date for Receipt of Pre-Qualification Documents
Receipt of pre-qualification documents shall close on 23rd July 2015 Pre-qualification documents shall be opened on the same day in the Senate Chamber, Ahmadu Bello House, Main Campus. Bidders or their representatives are invited to be present at the opening.
(7.0) Please Note
(a) Submission of pre-qualification documents to Usmanu Danfodiyo University, Sokoto is neither a commitment nor an obligation to award contract to any particular HMO.
(b) Advertisement for pre-qualification should not be construed as a commitment on the part of Usmanu Danfodiyo University, Sokoto nor shall it entitle any HMO to make any claim whatsoever or seek any indemnity from Usmanu Danfodiyo University, Sokoto.
(c) Due diligence shall be followed as all documents submitted would be verified and HMO’s office may be visited for verification. Any discrepancies found would summarily disqualify the HMO.
