Request for Proposals
(A) Background
Pro-Health International with funding from the Centre for Disease Control (CDC) through the Integrated Program for Sustainable Action Against HIV/AIDS in Nigeria (IPSAN) project is supporting the Government of Nigeria (GON) to enhance sustained cross-sectional integration of HIV/AIDS services by building Nigerian capacity to deliver sustainable high quality, comprehensive, prevention, treatment, care and related services in the three states of FCT, Nassarawa and Plateau. Pro-Health/IPSAN is seeking a contractor for the printing of her Monitoring and Evaluation Tools.
(B) Scope of Work
(1) The contractor will produce sample copy(ies) of tools contracted to it for printing approval if considered for this contract.
(2) The contractor shall be responsible for printing and delivery of all tools contracted to it.
(C) Request for Bidding Documents
(1) Interested contractors are requested to pick up the bid documents from the Administrative Manager at the understated address.
(D) Submission of Proposal
Complete proposals must include the following:
(i) Detailed company profile with registered address, functional email address, telephone numbers, and point of contact for the organization.
(ii) Evidence of company registration (i.e. Certificate of incorporation)
(iii) Audited financial statements for the past 3yrs
(iv) Tax Clearance Certificate for the past the 3yrs
(v) Evidence of experience in previous printing of similar tools
(vi) Recommendation letter from a past or present client
(vii) Bidders must submit requests for clarification concerning this bidding in writing via email to: prohealthprocurement@gmail.com not later than 4:00pm on Wednesday February 4th 2015.
NOTE: that your proposal shall strictly be evaluated based on adequacy of information provided. Proposals must be submitted in a sealed envelope on or before 12.00noon, Friday February 6th 2015. Envelope should be labeled “Proposal for Printing of Tools” and addressed to:
